OSU - Opportunities to Take Short Survey for Families of Children with I/DD

This is a survey for families of school-aged children (ages 6-22) with intellectual or developmental disabilities that we created in response to rapidly changing circumstances due to the COVID-19 pandemic.

Your families’ input about their experiences during this time is vital for practitioners, educators, and others to understand how we can better support them. We are asking families to complete this survey at three time points (once every 2 months). Families who complete the survey will be entered into a lottery and will be eligible to win a $20 Amazon gift card at each time point. 

Please note, our survey is currently only offered in English, but we will be getting out a Spanish translated survey within the next few weeks. We will follow up when the Spanish-translated survey is available.

If you answered YES to the above questions, please participate in our study by taking a short survey online at three time points (once every 2 months). Your survey responses MATTER. They will help us determine how to support families during this crisis. You will also be entered into a raffle to win a $20 Amazon gift card each time you take the survey.
