Researchers from Wingate University would like to learn more about the school-based telehealth services you and your child received during the COVID-19 Pandemic. This survey should take you 20 minutes to complete. You do not have to take this survey, but it will help us with our research if you do. Please consider taking this survey if your child: 1. was receiving school based physical therapy services before the COVID-19 Pandemic; 2. received at least 10 hours of telehealth school based physical therapy services during the COVID-19 Pandemic; 3. received services that were live and not taped.
